From our CHRODIS Partners: The CHRODIS PLUS Joint Action is a high-level response by the EU to alleviate the burden of chronic diseases in the EU Member States and beyond. The 36 month-long project is now in its 16th month with the first solid results, including completed national policy dialogues, ground set for the implementation projects, and the project represented at international and national conferences. This Newsletter has a special focus of one of the most important elements of the Joint Action: the implementation projects. Across Europe, at 21 locations our implementation projects have made significant progress. In this newsletter – based on an implementation project in Granada, Andalusia, – we present the key steps of how these implementation projects bring a new healthcare model into a region or country and can result in an innovation process across the healthcare system.
