Transforming Healthcare Through 3H+: A Call for Collaboration
The European Health Futures Forum (EHFF) has developed and tested an innovative approach—3H+—to catalyze transformational change in healthcare systems across Europe. This method responds to the long-recognized crisis in healthcare: a fragmented, reactive, and unsustainable system increasingly misaligned with people’s needs, environmental realities, and economic pressures.
At the heart of 3H+ is the Three Horizons framework, which maps the journey from today’s failing systems (Horizon 1), through transitional innovations (Horizon 2), to a resilient, community-centered, health-literate future (Horizon 3).
EHFF’s foresight process, supported by the Johnson & Johnson Foundation, gathered over 600 insights from experts across Europe, revealing both system failures and signs of emerging transformation.
What makes 3H+ unique is its integration of practical tools for implementation. These include:
- Stakeholder mapping to identify roles, strategies, and influence;
- Gap analysis to surface unmet needs and opportunities for innovation;
- A Transformational Catalyst model that supports collective sensemaking, action, and learning.
EHFF works as a “transformation catalyst”—a coordination hub currently also exploring DAO (Decentralized Autonomous Organization) principles to operate transparently, inclusively, and effectively across boundaries. This model bridges the trust gap between institutions and communities by combining networked legitimacy, real-time insights, and participatory governance.
The outcomes are clear: a scalable, adaptable methodology for healthcare transformation that is evidence-informed, systems-oriented, and action-driven. EHFF’s 3H+ has already sparked pilot initiatives, cross-sector dialogues, and deeper integration with broader movements like the Wellbeing Economy.

The window for change is open, but time is short. We invite researchers, practitioners, funders, policymakers, and community leaders to:
- Explore and adapt 3H+ tools for your context
- Collaborate in co-designing initiatives that bridge vision and action
- Participate in the EHFF network and help shape the future of health systems in Europe and beyond
Together, we can move from reactive care to resilient, regenerative systems that support people and planet. The future is emerging—let’s build it, deliberately and together.
